For the people families turn to

Let’s make the next transition easier to navigate.

A local point of contact for senior living searches and the practical housing questions that come with them. Connect directly with Zack McCarthy in Spokane and North Idaho.

When to connect

A family needs a clear starting point.

A family may be comparing care settings, preparing for a move, planning ahead, or working through a discharge with its care team. TruePath helps organize the search around assessed needs, budget, location, and timing.

Care and discharge teams

Hospital and rehab social workers, case managers, discharge planners, hospice teams, and geriatric care managers: discuss the family's search and how we can coordinate next steps.

Evaluators and home care

Qualified nurses and other evaluators, home health and home care professionals, caregivers, and therapists: introduce your services, scope, service area, and availability.

Housing and planning partners

Senior living teams, elder law attorneys, financial advisors, senior move managers, and downsizing specialists: help build a clearer path around the family's wider needs.

A simple introduction

Start with a call. Coordinate with consent.

Use the family's preferred introduction method. Please do not email medical records or identifying clinical information to make an initial connection.

01

Get acquainted

Call Zack or schedule a professional introduction. We can first discuss your services and general referral needs without sharing a person's private details.

02

Let the family choose

With their permission, introduce the family or give them TruePath's contact information. The family decides whether to engage and what information to share.

03

Agree on the next steps

Clarify timing, the right contacts, assessment requirements, and the family's priorities. Share coordination updates only as authorized by the family.

Clear roles

Independent expertise. Family choice.

TruePath provides referral and transition guidance. Independent qualified professionals conduct clinical assessments, and care providers determine admission and whether they can meet a person's needs. Families choose the professionals they engage.

Placement guidance is offered at no cost to families; contracted providers pay a referral fee for applicable placements. We explain compensation before proceeding. Real estate services through Keller Williams are separate and optional.
