Zack McCarthy
Founder, TruePath TransitionsMeet your local point of contact
I founded TruePath Transitions to help families organize the questions, compare senior living options, and coordinate the practical details of a move. We start with what matters to your family: care needs, budget, location, timing, and the person behind those needs.
When a home is part of the decision, I can also help you understand your real estate options through Keller Williams. You choose the professionals you work with, and you do not need to list a home with me to receive TruePath’s placement guidance.
For clinical care needs, I help families connect with qualified independent professionals. Together, we can build a clearer starting point for the search.

Our senior living referral and placement guidance is provided at no cost to families. When a placement is made with a contracted community or adult family home, that provider pays TruePath a referral fee. We explain the applicable agreement and compensation before moving forward. Independent assessments, moving services, legal advice, and other outside services may have their own fees. Real estate services are separately agreed through Keller Williams.
No. Placement guidance does not require a home sale or a real estate agreement. If housing is part of your plan, Zack can explain the options. Your family chooses whether to sell and which professionals to hire.
Absolutely. Call (509) 844-3204 or book a free introductory conversation. You do not need to know the right care setting or have every detail ready.
Qualified professionals and the prospective care provider handle clinical assessments and admission decisions. TruePath helps organize the search around those needs and can help you connect with an independent evaluator when appropriate.
Tell Zack your timing at the start. We can help organize questions and explore options whether you are planning ahead or working with a hospital or rehab team. Availability, assessment requirements, and acceptance vary by provider; placement or a specific timeline cannot be guaranteed.
TruePath helps narrow options, coordinate tours, prepare questions, and organize next steps. We discuss the support your family needs at the beginning and stay in touch through the transition.
